Qualifications

Responsibilities

• Investigates and reports abuse, neglect, or exploitation of adults or children with impairments to determine protective needs.
• Implements service plans and coordinates services to reduce risk and increase autonomy.
• Identifies risk factors and monitors progress of service plans with clients and families.
• Documents services, contacts, observations, progress in client files and databases.
• Participates in multidisciplinary teams to strategize client services and educate external entities about agency programs.
