Qualifications

• Current and unrestricted California PA or NP license.
• Current national certification as PA or NP.
• Current DEA registration, if required for assigned responsibilities.
• Current BLS certification.
• Strong clinical assessment and decision‑making skills.
• Excellent communication, documentation, and care‑coordination skills.
• Ability to work independently while collaborating with supervising physicians and facility teams.

Responsibilities

• Provide medical care and routine rounding for patients in assigned skilled nursing facilities.
• Perform comprehensive patient assessments, histories, and physical examinations.
• Manage acute and chronic medical conditions within the provider's scope of practice and credentials.
• Complete admission evaluations, follow-up visits, and discharge-related care as assigned.
• Develop, implement, and monitor individualized treatment plans.
• Coordinate specialty consultations, referrals, and hospital transfers when medically necessary.
• Collaborate with physicians, nursing staff, rehabilitation teams, and facility leadership.
