Arrae Health is seeking a compassionate SNFist PA-C or NP to provide medical care in skilled nursing facilities in Palm Springs, CA. The provider will manage medically complex adult and geriatric patients, working closely with physicians, nursing staff, and rehabilitation teams.

Requirements include an active California PA or NP license, national certification, DEA registration if needed, and strong clinical assessment and communication skills.

• Must hold active California PA or NP license.
• National certification as PA-C or NP.
• Current DEA registration, if required for duties.
• BL S certification and ability to provide patient care.
• Strong clinical assessment and decision-making skills.

Responsabilidades

• Provide medical care and routine rounding for SNF residents.
• Perform comprehensive history, exams, and follow-up visits.
• Manage acute and chronic conditions within scope and credentials.
• Complete admission evaluations, follow-ups, and discharge-related care.
• Develop, implement, and monitor individualized treatment plans.
• Order and interpret labs and imaging; coordinate specialty referrals.
