Job summary

Ohana Pacific Health is seeking a Social Services Assistant in Hilo, HI to work with interdisciplinary teams assessing Kupuna's psychosocial, emotional, environmental, spiritual, and support needs to create care plans.

Responsibilities include counseling, crisis intervention, rounds, and coordinating POA/guardianship processes to ensure proper representation at admission.

Qualifications

• Bachelor's degree in Social Work preferred/required.
• Membership in NASW and ACSW preferred.
• Hawaii State Social Work license or eligibility preferred.
• Minimum 1 year of social services or long-term care experience preferred.

Responsibilities

• Assist with counseling and crisis intervention to support coping with illness, disability, death, and relationship issues.
• Make daily rounds; review social service and loss logs.
• Complete initial assessments, state/federal timed assessments, and care plans.
• Identify guests needing POA, surrogates or guardianship and coordinate representation.

The Position:

We're looking for a caring and compassionate Social Services Assistant who works in conjunction with the interdisciplinary team to identify and assess our Kupuna's psychosocial, emotional, environmental, spiritual, and support needs to develop a plan of care. Responsibilities include:

• Assists with counseling and crisis intervention to facilitate effective coping/problem solving strategies in relation to illness/disability, death, and/or emotional and relationship problems.
• Makes daily rounds to all units and reviews communication logs, social service logs and loss logs.
• Completes the process for Social Services to include: initial assessments, state/federal timed assessments, and care plans.
• Identifies guests who need to Obtain POA, Surrogates or Guardianship and assists in coordinating process to ensure all guests have a representative on admission or within 1 month of admission.

Requirements:

• Bachelor's degree in Social Work preferred.
• Membership in National Association of Social Workers and Academy of Certified Social Work preferred.
• Licensure/ Hawaii State Social Work license or eligible preferred.
• Minimum of 1 year of experience providing social services or in long term care preferred.
