Job summary

Ohana Pacific Health is seeking a Social Services Manager at the Daniel K. Akaka State Veterans Home in Kapolei, HI. The role plans, coordinates, and implements the overall emotional and social needs program for residents.

You will provide counseling, case management, and consultation to team members and community partners to address residents' needs and rights within a compassionate, mission-driven environment.

Qualifications

• Master's Degree in Social Work or combination of education and experience.
• Experience in a health care setting preferred, supervisory experience preferred.
• Licensed by the State of Hawaii as a Social Worker.

Responsibilities

• Informs residents and families about residents' rights.
• Provides individual and family counseling and case management services when necessary.
• Provides consultation to team, community, agencies, etc. to develop social service programs.

The Position:

We're looking for a passionate Social Services Manager to plan, coordinate and implement the overall emotional and social needs program for residents. This position encompasses:

• Assures that all residents and families are informed fully concerning "residents rights"
• Provides individual and family counseling and case management services to individuals and families when necessary.
• Provides consultation to members of the team, community, agencies, etc. in efforts to solve the needs and problems of the residents through the development of social service programs.

Requirements:

• Master's Degree in Social Work or combination of education and experience.
• Experience in a health care setting preferred, supervisory experience preferred.
• Licensed by the State of Hawaii as a Social Worker
