Job summary

Enhance Rehabilitation in the United States seeks qualified Physical Therapists to join our growing team in SNF settings. You will evaluate and treat patients following an individualized plan of care while collaborating with facility staff to ensure the highest quality of care.

Responsibilities include supervising Physical Therapist Assistants, re-evaluating patient progress, updating plans and documentation such as evaluations, re-evaluations, discharges and daily progress notes.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
