Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care, provide exceptional patient care, and collaborate with facility staff to ensure top-quality outcomes.

You will also supervise Physical Therapist Assistants, re-evaluate progress, adjust plans of care, and maintain complete, timely documentation including evaluations, re-evaluations, discharges and daily progress

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
