Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in the SNF setting, providing high-quality patient care. You will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff, supervise assistants, and document evaluations, re-evaluations, discharges and progress notes while communicating progress and outcomes to patients and families.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
