Flagstar Rehab is seeking Physical Therapists for Skilled Nursing Facilities in NYC and surrounding areas. Join a diverse, multi-disciplinary team to evaluate, treat, and educate patients to improve mobility and cognition.

We offer direct employment or independent contractor options, with weekly pay, benefits options, and support to establish your own practice. Flexible schedules including full-time, part-time, per diem, and weekend coverage are available.

Qualifications

• NYS Physical Therapist license and registration.
• CPR certification.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to SNF patients with conditions such as TBI, CVA, stroke, Parkinson's, MS, cancer, and orthopedic concerns.
• Monitor and document patient progress using EMR software.
