Flagstar Rehab is seeking Physical Therapists to join our skilled nursing facility collaborations in NYC and surrounding areas. You will evaluate patients, develop individualized treatment plans, and supervise PTAs while delivering high-quality therapy to SNF residents.

Applicants must hold an active NYS PT license, CPR certification, and be proficient with EMR software. We offer flexible schedules, competitive hourly rates, and weekly pay with direct deposit.

Qualifications

• Licensed PT in New York with current registration.
• CPR certification is required.
• Experience or ability to work with EMR systems.

Responsibilities

• Perform evaluations and create individualized PT treatment plans.
• Supervise PTAs on a daily basis.
• Deliver therapy services in SNF settings for varied diagnoses.
• Monitor and document patient progress using EMR.
