Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care.

As part of our growing team, you will collaborate with facility staff, provide patient education, document progress and contribute to achieving measurable goals and positive outcomes for patients and families.

Qualifications

• Responsibilities include evaluating and treating patients in a SNF setting following an individualized plan of care.
• Documenting evaluations, progress notes, re-evaluations, discharges, and daily progress.
• Supervising Occupational Therapy Assistants and collaborating with facility staff.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
