Job summary

Enhance Rehabilitation in Woodstock, IL is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure excellent patient outcomes.

You will re-evaluate progress, adjust plans, document evaluations and discharge notes, educate patients and families, and supervise Occupational Therapy Assistants.

Qualifications

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.

Responsibilities

• Assess and treat patients in SNF following individualized plan of care.
• Develop and monitor goals with measurable outcomes.
• Educate patients and families on injury, progress and outcomes.
• Re-evaluate progress and adjust plans accordingly.
• Complete timely documentation including evaluations and discharge notes.
• Supervise Occupational Therapy Assistants and collaborate with staff.
