Job summary

Burlington Health and Rehab is seeking a Speech Language Pathologist (SLP) to lead and deliver comprehensive speech therapy services, plan treatment, document progress, and collaborate with residents, families, and staff to maintain high standards of care.

This full-time role offers flexible scheduling or Per Diem options, a $3000 sign-on bonus, competitive benefits, ongoing professional development, and a supportive team environment under Packers Staffing.

Qualifications

• Must have Master's degree in Speech Language Pathology.
• CCC (Clinical Competency) certification required.
• Active SLP license in practicing state(s) required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with target lists, short-term and long-term goals.
• Instruct patient, family, and staff to ensure carryover of therapeutic remediation and goals in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of proper FEES/MBS protocols with coordination of site programs.
• Participate in screening protocols for establishing residents' needs in a timely and clinically excellent manner.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency (CCC)

Active SLP license in state of practice

Continuing education to maintain licensure
