Job summary

Pro Rehab is hiring a Speech Language Pathologist to float between Cokato and Dassel in Minnesota. The role offers full‑time or part‑time schedules, competitive wages, mileage reimbursement, and a $2,500 bonus paid over 18 months. Requires CCC‑SLP certification and MN license.

The SLP will assess and diagnose communication disorders, develop treatment plans, provide therapy, and educate patients and families while collaborating with a healthcare team to ensure quality care across the West Region.

Qualifications

• Graduate from an Accredited Speech Therapy Program with a bachelor’s degree.
• Current MN Speech Therapist License is required.
• Experience with Homecare Homebase EMR is helpful but will train.
• Experience with OASIS is preferred or willing to be trained.
• Strong knowledge of state, federal, and local Home Healthcare regulations.
• Ability to develop care plans and discharge planning strategies.
• Excellent communication skills to educate patients and families on therapy techniques.
• Ability to perform the essential functions of the job with or without accommodations.

Responsibilities

• Assess and diagnose communication disorders across speech, language, voice, fluency, and swallowing.
• Develop and implement personalized treatment plans to improve functional deficits.
• Provide speech and language therapy through targeted exercises and interventions.
• Educate and support patients and families on strategies for effective communication and daily life.
• Monitor progress, document outcomes, and adjust therapy plans as needed.
• Collaborate with doctors, nurses, and other therapists to ensure comprehensive patient care.
