Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) for Skilled Nursing Facilities in NYC and surrounding areas. You will assess speech and swallow function, develop and adjust plans of care, and educate patients and families to improve daily living activities.

Work involves collaboration with a multidisciplinary team and delivering therapy in SNF settings for conditions such as stroke, TBI, Parkinson’s, and MS.

Qualifications

• CFY not accepted.
• Current NYS SLP License and Registration.
• CCC Certification is required.
• CPR Certificate.
• EMR Competence.
• State required medical documentation for work in a Skilled Nursing Facility.
• Proof of Identity and Work Authorization.
• Malpractice/Liability Insurance if applicable.

Responsibilities

• Provide efficient speech and swallow evaluations and/or screens.
• Develop plan of care and manage meal textures as needed.
• Deliver quality therapy services to SNF patients with conditions like stroke, TBI, Parkinson’s, MS, cancer.
• Monitor and document patient progress using EMR.

Speech Language Pathologist (SLP) in Skilled Nursing Facility

Now Staffing Speech Language Pathologists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab SLPs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the speech and swallow capabilities of their patients and improve ability to function in their daily lives. Not accepting Clinical Fellows (CFY) at this time.

Flagstar Rehab offers the opportunity for Direct Employment with our agency or to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free-of-charge.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
