Job summary

Relient Health seeks a full-time Speech Language Pathologist (SLP) for a Skilled Nursing Facility in Vandalia, OH. You will evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders, and develop individualized plans in a collaborative rehab team.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current OH SLP license (or eligibility).
• CCC-SLP preferred, but CFs will be considered when supervision is available.
• Strong interpersonal and documentation skills.

Responsibilities

• Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders
• Develop and implement individualized treatment plans
• Document patient progress using EMR systems
• Collaborate with PTs, OTs, nurses, and physicians to deliver coordinated care
• Educate patients, families, and staff on treatment goals and progress
