Job summary

Stern Therapy Consultants is seeking a PRN Speech-Language Pathologist at Vantage Health in Wakefield, MA, to provide individualized services for communication, cognition, and swallowing needs in a skilled nursing rehabilitation setting. The role will evaluate, plan, and deliver therapy, develop measurable goals, and track outcomes to support discharge planning.

A MA license is required; PRN coverage is arranged around facility needs.

Qualifications

• Strong documentation skills and the ability to explain recommendations clearly.

Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing abilities to determine appropriate treatment.
• Develop measurable goals that address residents' functional challenges and personal priorities.
• Deliver therapy focused on expressing needs, understanding conversations, recalling information, and managing everyday cognitive tasks.
• Provide dysphagia treatment and communicate swallowing recommendations to staff.
• Educate residents and caregivers on techniques that support communication and carryover of therapy strategies.
• Track outcomes, update plans of care, and complete documentation supporting discharge planning.

Stern Therapy Consultants is seeking a PRN Speech-Language Pathologist at Vantage Health in Wakefield, MA, providing individualized services for communication, cognition, and swallowing needs in a skilled nursing rehabilitation setting.

What You'll Do

• Evaluate speech, language, cognitive-communication, and swallowing abilities to determine appropriate treatment.
• Develop measurable goals that address residents' functional challenges and personal priorities.
• Deliver therapy focused on expressing needs, understanding conversations, recalling information, and managing everyday cognitive tasks.
• Provide dysphagia treatment and communicate clinically appropriate swallowing recommendations to nursing and dietary staff.
• Educate residents and caregivers on techniques that support communication and carryover of therapy strategies.
• Track outcomes, update plans of care, and complete documentation that supports coordinated treatment and discharge planning.
