Job summary

COUNTRY OAKS CARE CENTER in Pomona, CA is seeking a Speech-Language Pathologist (SLP) to provide, direct and supervise all aspects of quality patient care in a post-acute environment.

The role requires a Master of Science in Speech/Language Pathology, current CA licensure, CPR certification, and 1 year of SLP experience (CFY candidates welcome). Preference for SNF/residential settings.

Qualifications

• Master of Science in Speech/Language Pathology.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• Students in their CFY (Clinical Fellowship Year) will be considered.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Ensures patient care follows the organization’s guidelines, professional standards and community needs, and complies with Federal and State guidelines.
• Serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers.
• Ensures cross-disciplinary coordination to provide for consistent patient care.
• Participates in quality assurance and customer service activities.
