Job summary

Vertis Therapy is seeking a full-time Speech-Language Pathologist (SLP) to join our rehabilitation team at Lake Meadows in Fishers, Indiana. You will evaluate and treat residents to improve communication, cognition, and swallowing, while building meaningful relationships and partnering with an interdisciplinary team.

The role emphasizes resident-centered care, independence, and high-quality outcomes within an assisted living/memory care setting.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited institution.
• CCC from ASHA or eligibility for CFY required/eligible.
• Valid state licensure or eligibility to obtain licensure as a Speech-Language Pathologist.

Responsibilities

• Evaluate and treat residents' communication, cognitive, and swallowing abilities and implement individualized treatment plans.
• Complete timely evaluations, progress notes, treatment documentation, and discharge summaries per requirements.
• Participate in team development meetings and collaborate with an interdisciplinary care team.
