Job summary

Life Care Centers of America is seeking a Speech Language Pathologist (SLP) to provide direct speech therapy in SNF settings. You will work with an interdisciplinary team to plan and implement therapy, document progress, and support patient-centered goals.

Required MS/MA in Speech-Language Pathology, ASHA CCCs or CFY eligibility, and active state licensure. CPR is needed on hire, with flexibility for orientation and continued professional development.

Qualifications

• MS or MA in Speech-Language Pathology required.
• ASHA CCCs or CFY eligibility and credentialing criteria must be met.
• Current state license in good standing required; CPR certification preferred.
• One year experience preferred; new grads welcome to apply.

Responsibilities

• Provide direct speech therapy rehab to patients per physician referral.
• Participate in interdisciplinary team planning and care.
• Document therapy progress in appropriate systems.
• Assist with patient and family education and discharge planning.
