Job summary

The Speech Language Pathologist at Trilogy Health Services – The Springs at Lafayette provides a full range of speech-language pathology services, including assessment, treatment planning, and therapeutic interventions in a collaborative interdisciplinary setting for residents. A Master’s degree in Speech-Language Pathology and current state licensure are required; CCC-SLP is preferred or required.

• Master’s degree in Speech-Language Pathology.
• Current state SLP licensure in Indiana.
• CCC-SLP preferred or required.
• CPR certification preferred.

Responsibilities

• Evaluates patients to identify speech, language, voice, and swallowing disorders.
• Develops and implements individualized therapy plans based on assessment findings.
• Provides therapy to improve communication and swallowing abilities.
• Educates patients and families about disorders and therapy strategies.
• Maintains accurate documentation of evaluations, progress notes, and plans.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
