Job summary

Trilogy Health Services is seeking a Speech-Language Pathologist to deliver comprehensive speech, language, voice, and swallowing services at Creasy Springs Health Campus in Lafayette, IN. You will assess, plan, and implement therapy, coordinate with the care team, and document progress in compliance with regulations.

The role emphasizes interdisciplinary collaboration, patient and family education, and ongoing documentation, with travel as needed and eligibility for benefits and weekly pay.

Qualifications

• Evaluates patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develops and implements individualized therapy plans based on assessment findings, setting measurable goals to address patients' specific needs.
• Provides therapy to improve patients’ communication skills, including articulation, language comprehension, voice modulation, and fluency, as well as swallowing and feeding abilities.
• Educates patients and their families about the nature of disorders, therapy techniques, and strategies to support progress and enhance communication and swallowing in daily life.
• Maintains accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans in compliance with facility policies and regulatory requirements.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

POSITION OVERVIEW

The Speech Language Pathologist (SLP), is responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

Education: Master Degree
Experience: 0-1 years

Licenses and Certifications:

Must have and maintain a current, valid state SLP licensure

Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred or required

Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
