Job summary

Resolve West Allis, a skilled nursing facility, seeks a compassionate Speech Language Pathologist to join our rehabilitation team to help residents improve communication, cognition, and swallowing.

You will conduct evaluations, develop treatment plans, and collaborate with physicians, nursing staff, and families in a resident-centered care approach. On-site position with competitive pay and full benefits.

Qualifications

• Must have a Master’s degree in Speech Language Pathology.
• CCC-SLP certification preferred but not required.
• Experience in skilled nursing, rehabilitation, or post-acute settings encouraged.
• Strong communication, organization, and documentation skills essential.
• Passion for resident-centered care and improving quality of life.

Responsibilities

• Provide speech therapy evaluations and develop individualized treatment plans.
• Assess and treat speech, language, cognitive, communication, and swallowing disorders.
• Implement evidence-based treatment interventions to maximize resident outcomes.
• Collaborate with physicians, nursing staff, therapy team, residents, and families.
• Maintain timely and accurate documentation in accordance with Medicare, state, and facility guidelines.
• Participate in care planning meetings and discharge planning activities.
• Educate residents, families, and staff regarding treatment plans and recommendations.
