PowerBack Rehabilitation is seeking a Skin Health Team Lead to champion skin integrity in our skilled nursing setting. The role drives wound care rounds, guides staff, and coordinates admissions to ensure optimal resident outcomes.

You will collaborate with nurses, therapists, and families, leveraging advanced wound training to improve care and documentation. A strong clinical background and leadership are essential.

Qualifications

• RN license required and active in state of practice.
• Completion of Skin Health Team Lead orientation within 90 days of hire.
• Completion of Wound Basics within 90 days of hire.
• Advanced Wound Training and national wound certification within 1 year of hire.

Responsibilities

• Lead weekly wound rounds with the interdisciplinary team and document care plans.
• Coordinate admissions with skin/wound needs for smooth transitions.
• Provide bedside training and coaching on skin and wound care best practices.
