Job summary

Genesis HealthCare is seeking a Skin Health Team Lead to champion skin integrity and wound care. You will lead interdisciplinary rounds, guide staff, and drive improvements in prevention and treatment while coordinating with nursing leadership for admissions and care plans.

The role emphasizes education, documentation, and timely assessment of all new skin/wound conditions, with a focus on quality outcomes for residents in our care facilities.

Qualifications

• RN license in the state where the position exists.
• Completion Skin Health Team Lead orientation within 90 days of hire.
• Completion of Advanced Wound Training and national wound certification within 1 year of hire.
• Knowledge of nursing home regulations and survey process; proficient with charting systems.

Responsibilities

• Lead weekly wound rounds and care planning.
• Coordinate admissions with skin/wound needs and prepare for new residents.
• Provide education and guidance on skin integrity and wound care to staff and families.
• Ensure staff competency and accurate documentation.
• Collaborate with interdisciplinary team to improve outcomes for residents.
• Manage specialty support surfaces and support audits and improvements.
• Drive root-cause analysis and process improvements for wound care.
