Job summary

Belleville Healthcare and Rehabilitation Center is seeking a licensed Physical Therapist to develop and implement individualized therapy plans for SNF patients, conducting assessments, treatments, and progress documentation. You will collaborate with physicians, nurses, and therapists to maximize functional goals and ensure high standards of care.

The role requires strong communication, organizational skills, and willingness to travel within the facility as needed; licensing and adherence to

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Current license to practice Physical Therapy in the state.
• Knowledge of state, federal and professional standards.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care based on thorough assessment.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Documents evaluations, treatment goals and plans; maintains prescriptions and records.
• Keeps up to date on professional literature and participates in continuing education.
