Job summary

Topaz Fiscal Services seeks a LPN/RN Unit Manager to oversee clinical operations on a skilled nursing unit. You will supervise CNAs, LPNs, and RNs, ensure accurate documentation, and collaborate with the IDT to deliver person-centered care.

You will lead staffing, ensure CMS and Georgia regulations are met, participate in quality improvement, and support MDS/RAI data coordination while fostering a culture of accountability and excellence.

Qualifications

• Active, unrestricted LPN or RN license issued by the Georgia Board of Nursing.
• ADN or higher degree preferred for RN applicants.
• Current CPR/BLS certification.
• IV certification required for LPN or willingness to obtain within 90 days of hire.
• Minimum 2 years of clinical nursing in long-term care or SNF setting.
• Minimum 1 year of charge nurse/ unit management experience preferred.
• Experience with PCC or equivalent SNF EHR preferred.

Responsibilities

• Supervise and direct nursing staff (CNAs, LPNs, RNs) on the unit across all shifts.
• Conduct and document comprehensive nursing assessments for new and current residents.
• Develop and update individualized care plans with the IDT.
• Monitor residents and initiate interventions with physician notification as needed.
• Oversee wound care, medication management, and treatment protocols.
• Participate in IDT meetings, care conferences, and family meetings.
