Topaz Fiscal Services LLC is seeking an LPN/RN Unit Manager to provide clinical oversight, staffing coordination, and day-to-day operations for a SNF nursing unit.

Ideal candidates have active GA licensure (LPN/RN), 2+ years in long-term care, and leadership experience. The role emphasizes quality improvement, regulatory compliance, and interdisciplinary collaboration with IDT and families.

Qualifications

• Active LPN or RN license in Georgia required.
• ADN or higher preferred for RN applicants.
• CPR/BLS certification required.
• IV certification for LPN or willingness to obtain within 90 days of hire.
• Minimum 2 years clinical nursing in LTC/SNF or post-acute setting.
• Minimum 1 year of charge nurse/supervisory experience preferred.

Responsibilities

• Supervise and direct nursing staff on the unit across all shifts.
• Conduct and document comprehensive nursing assessments for new admissions and current residents.
• Develop, review, and update individualized care plans with the interdisciplinary team (IDT).
• Monitor residents for changes; initiate interventions and notify physicians as needed.
• Oversee wound care, medication management, and treatment protocols.
• Participate in IDT meetings, care conferences, and family meetings.
• Collaborate with therapy, social services, dietary, and activities for holistic care.
• Assist the DON with unit-level scheduling and staffing considerations.
• Ensure regulatory documentation and CMS compliance across the unit.
