• Evaluate patients based on needs, limitations and desires.
• Facilitate care plan meetings and discharge planning.
• Completing MDS social work sections.
• Address patient concerns and goals while communicating with patients and case managers.
• Build rapport with patients and families.
• Coordinate care with Nursing and rehab departments.
• Develop care plans for patients.
• Maintain knowledge of long-term care regulations and documentation requirements.
• Audit psychology paperwork, hospital discharges, room changes, PASRR and advanced directives.
