Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in South Pasadena, FL. This part-time or PRN role requires a valid Physical Therapy license in Florida and a Bachelor of Science in Physical Therapy.

Responsibilities include evaluating residents, developing treatment plans, documenting in CMS-compliant systems, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct routine screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
