Job summary

Enhance Rehabilitation in Illinois is seeking qualified Occupational Therapist candidates to join our dynamic team. In this SNF setting you will evaluate and treat patients, develop and implement plans of care, and collaborate with facility staff to ensure high-quality outcomes.

You will supervise Occupational Therapy Assistants, complete timely documentation, and educate patients and families about progress and outcomes.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
