You will collaborate with the facility’s care team to oversee both short- and long-term residents, handling admissions, discharges, and ongoing assessments.

You’ll conduct advanced care planning, comprehensive evaluations, and prescribe treatments as appropriate.

Qualifications

• Active Texas APN licensure and DEA license required.
• Previous nursing or clinical experience.
• Strong communication and teamwork skills.
• Master’s degree in Nursing required.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations for changes in condition.
• Lead advanced care planning and comprehensive assessments.
