Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to join our SNF team. You will act as a clinical leader, collaborating with the facility care team to supervise admissions, discharges, monthly stability checks, and evaluations for changes in condition.

You will handle advanced care planning, comprehensive assessments, and prescribing treatments, ensuring high-quality resident care and seamless coordination with clinicians.

Qualifications

• Active Texas APN licensure and DEA license required.
• Master’s degree in Nursing.
• Previous nursing or clinical experience.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations for changes in condition.
• Lead advanced care planning and comprehensive assessments.
• Prescribe treatments as appropriate.
• Collaborate with facility care team to ensure quality of care.
