Job summary

Humana Inc. is seeking a Home Health Registered Nurse to deliver one-on-one care in patients' homes. You will develop individualized care plans, administer medications, monitor progress, and collaborate with physicians and the Clinical Manager.

Qualifications

• Active, unencumbered RN license.
• Med-surg/ICU/ER or acute care experience.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Provide skilled nursing care to patients in their homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate with healthcare professionals.
• Report progress to patient’s physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
