Job summary

Humana Inc. is seeking a Home Health Registered Nurse to deliver personalized, one-on-one care in patients' homes, reporting to the Clinical Manager. You will manage care plans, educate families, and coordinate with clinicians to support recovery.

Responsibilities include medication administration, wound care, and patient assessments, while supervising Home Health Aides and ensuring accurate documentation. A strong team approach and dependable transportation are required.

Qualifications

• Med-surg/ICU/ER/acute care experience.
• Current unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with healthcare professionals.
• Valid driver's license, auto insurance and transportation.

Responsibilities

• Provide skilled nursing care at patients' homes, administer meds, wound care, and assessments.
• Develop individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management and treatments.
• Maintain records and coordinate with healthcare professionals.
• Oversee Home Health Aide documentation.
