Job summary

Humana Inc. is seeking a Home Health Registered Nurse to provide one-on-one skilled care in patients' homes under the guidance of the Clinical Manager. You will develop care plans, administer medications, and monitor progress to support recovery and independence.

Reporting to the Clinical Manager, you will collaborate with physicians and a care team, educate families, oversee Home Health Aides, and ensure accurate documentation while maintaining compliance and patient safety.

Qualifications

• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, wound care, and patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
