BrightStar Care in Indianapolis is seeking a Skilled Home Health Care Manager (RN) to lead clinical operations, support care transitions, and grow skilled home health services through strong client relations and strategic partnerships.

You will collaborate with hospitals, physicians, case managers, and payer sources to promote high-quality care, while guiding an interdisciplinary team and ensuring federal participation requirements are met.

Qualifications

• Active RN license in good standing.
• 1–3 years of experience in home health, post-acute care, case management, or healthcare operations.
• Knowledge of skilled home health services and federal conditions of participation.
• Experience developing professional healthcare relationships and referral networks.
• Strong communication, organizational, leadership, and relationship management skills.

Responsibilities

• Care Delivery and Care Transition Support: Collaborate with healthcare providers to ensure smooth transitions to home while supporting positive patient outcomes.
• Collaboration & Leadership: Partner with operations and clinical teams to support growth objectives, quality outcomes, and exceptional customer service.
• Referral Relationship Management: Develop and maintain strong relationships with discharge planners, case managers, social workers, physicians, managed care organizations, and other payer source representatives.
• National Account Development: Support the growth and expansion of national account relationships, healthcare partnerships, and referral networks.
• Strategic Partnership Building: Represent BrightStar Care in meetings, networking events, and business development activities to strengthen market presence and service utilization.
• Skilled Home Health Services Growth: Promote BrightStar Care's skilled home health services and identify opportunities to improve clinical outcomes and client satisfaction.

The Skilled Home Health Care Manager (RN) is responsible for supporting and leading the growth of skilled home health services through clinical expertise, effective client relations, referral relationship management, and strategic partnership development. This role serves as a key resource for agency clinicians and caregivers, as well as hospitals, physicians, case managers, managed care organizations, and national account partners, to promote high-quality skilled home health services.

The ideal candidate is a Registered Nurse with a strong understanding of home health clinical services and operations, federal conditions of participation, care transitions, risk management, and post-acute patient management.

Key Responsibilities

• Care Delivery and Care Transition Support: Collaborate with healthcare providers to ensure smooth transitions to home while supporting positive patient outcomes.

Qualifications

• Active Registered Nurse (RN) license in good standing.
• Minimum of 1 to 3 years of experience in home health, post-acute care, case management, or healthcare operations.
• Knowledge of skilled home health services and federal conditions of participation.
• Understanding of skilled home health care, care transitions, and home health eligibility requirements.
• Experience developing professional healthcare relationships and referral networks.
• Strong communication, organizational, leadership, and relationship management skills.
