Job summary

UH Cleveland Medical Center in Cleveland, Ohio seeks a Surgical ICU Clinical Pharmacy Specialist to provide dedicated pharmacotherapy services to critically ill surgical patients and to collaborate with the ICU team to optimize therapy.

The role includes rounds, monitoring, formulary stewardship, guideline development, and teaching; it offers opportunities for research and professional growth within a leading academic medical center.

Qualifications

• PharmD required and Ohio licensure or eligibility.
• PGY1 residency preferred; PGY2 CC residency strongly preferred or equivalent experience.

Responsibilities

• Serve as pharmacotherapy expert for critically ill surgical patients in the SICU.
• Participate in daily multidisciplinary rounds and contribute to care plans.
• Provide TDM and pharmacokinetic consultation for critically ill patients.
• Develop and maintain clinical policies, guidelines, and protocols.
• Lead quality improvement, medication safety, and stewardship initiatives.
• Educate pharmacists, physicians, nurses, residents, and students.
