Job summary

Qualifications

• Pharmacy doctorate with ASHP residency preferred, or 3–5 years in the clinical area.
• Pharmacist license in Ohio required.

Responsibilities

• Provide clinical pharmacy services including drug information, therapy monitoring, patient-specific consultations.
• Investigate, research and report adverse drug reactions.
• Participate in Medication Safety and Therapeutics committee processes and formulary maintenance.
• Serve as a resource for clinical pharmacists and other healthcare professionals.
• Deliver continuing education to clinical pharmacist staff.
• Document clinical activities and outcomes.
• Act as a preceptor for pharmacy residents and students.
• Participate in local and system committees related to critical care pharmacotherapy.
