Job summary

The Cancer & Hematology Centers is seeking a Sickle Cell Program Coordinator (RN) to coordinate comprehensive, patient-centered care for individuals living with sickle cell disease across the care continuum.

You will collaborate with hematology, primary care, infusion and social work teams to ensure timely, evidence-based and equitable care while advancing quality and access for patients and families.

Qualifications

  • Current Registered Nurse (RN) license in state of practice.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum 3–5 years clinical nursing experience.
  • Experience in hematology, oncology, infusion, or chronic disease management.
  • In person only.

Responsibilities

  • Coordinate comprehensive, patient-centered care for individuals with sickle cell disease.
  • Serve as primary point of contact for patients and coordinate outpatient, inpatient and infusion services.
  • Facilitate transitions of care (ED to home, inpatient to outpatient).
  • Monitor high-risk patients and intervene to prevent avoidable admissions.
  • Collaborate with hematology providers on disease management plans and care pathways.
  • Educate patients and families on disease, medications, pain management, and emergency signs.
  • Identify social determinants of health and connect patients to resources.
