Job summary

Collage Rehabilitation Partners is seeking a Full-Time Shift Coordinator for our residential program in Lilburn, GA. You will apply rehabilitation strategies and provide minimum to maximum assistance to promote daily living, recreational activities, and vocational goals.

Schedule is Monday–Friday 11pm–7am with a Thursday meeting at 2pm; training sessions occur during the day. Responsibilities include supervising staff, coordinating clinical direction, and ensuring resident progress and safety.

Qualifications

• High school diploma or GED required.
• At least 18 years old.
• Valid driver’s license required.
• Ability to lift 50 lbs.

Responsibilities

• Models appropriate service delivery for residents and mentors Life Skills Trainers in the delivery of services to residents.
• Communicates information to program management to ensure staffing models meet resident needs.
• Assume the role of primary contact for clinical direction for the shift; liaison for clinical and operational on‑call.
• Assists and guides residents in activities of daily living, community living skills, rehabilitation, and vocational goals.

We are searching for a Full-Time Shift Coordinator to work in our residential program located in Lilburn, GA. This position is responsible for the application of client rehabilitation strategies, and maintains responsibility for providing minimum to maximum assistance and/or supervision to promote our clients’ success in tasks associated with daily living, recreational activities, and vocational pursuits.

Schedule: Monday - Friday 11pm-7am Must also be available to attend a meeting on Thursdays at 2pm

Training: Must be available to attend trainings during the day, especially weekdays

Duties:

• Models appropriate service delivery for residents and mentors Life Skills Trainers in the delivery of services to residents
• Communicates information to program management to ensure that appropriate staffing models are in place to address resident needs
• Assume the role of primary contact for clinical direction for the shift. Primary liaison for clinical and operational on - call
• Assists, guides, and encourages independence when working with clients in the areas of activities of daily living, instrumental community-based living skills, and ongoing rehabilitation and supported living goals and outcomes. These areas of activities can include, but are not limited to, medical and health related issues, medication management, physical activities, cognitive activities, productive activities, therapeutic activities, recreational/leisure activities, and vocational or avocational activities

Requirements:

• A high school diploma or GED is required
• Must be at least 18 years of age
• Must have a valid driver's license and responsible driving behaviors must be maintained
• Ability to lift 50lbs
