Job summary

ArchWell Health is seeking a licensed Clinical Social Worker to join our patient-centered care team in Charlotte, NC. You will provide social work services to provide access, advocacy, and support for seniors within a primary care setting.

The role emphasizes care coordination, behavioral health assessments, short-term counseling, and collaboration with clinicians to determine appropriate levels of care, while documenting encounters in clinical systems and promoting compassionate, equitable care.

Qualifications

• MSW required and licensed LCSW in good standing.
• Experience with social work in clinical settings.
• Ability to plan and implement social services assessments.
• Experience with care coordination and case management.

Responsibilities

• Develop relationships and collaborate with primary care teams to assess members.
• Plan, coordinate, manage and implement support packages.
• Navigate managed care plans for community services and programs.
• Case management for social and behavioral care to enable self-management.
• Document patient encounters and counseling in management systems.
• Identify methods to improve approaches based on feedback and surveys.
• Support advance care transitions with members and families.
• Assist members with access to prescription programs and other benefits.
• Collaborate to assess level of care and screen for MH and substance use disorders.
• Develop care plans and provide short-term counseling or referrals.
• Documentation of encounters and counseling sessions.

Duties/Responsibilities:

• Assist members with access to state-based prescription programs and other benefits
• Collaboratewiththeprimarycareteamstoconductbehavioralhealthassessmentstodeterminethe appropriate level of care for each member; screen and access each member for commonmental health and substanceusedisorders
• Develop care plans and provide short-term counseling or facilitate referrals, including warm handoffsfortelehealth visitsor referralstoexternalspecialists
• Accurateandtimelydocumentationofpatientencountersandcounselingsessionsinallclinicalmanagementsystems
• Proactivelyidentifymethodstoimprove ArchWellHealth’sapproachbasedonfeedbackandregularlyconductedsurveys

Required Skills/Abilities:

• Experiencesupportingpatientswithco-occurringbehavioralhealthandsubstanceabuseissues

• Working knowledge of differential diagnosis of common mental health and/or substance usedisorders
