Responsibilities

• Communicate effectively with interdisciplinary teams to meet resident needs.
• Participate in CST meetings to discuss resident progress and needs.
• Assist with safe transfers, positioning, and mobility using equipment and techniques.

DOE. + $1.50 Shift Differential **Schedule:** Part-Time 0.6 FTE 45hrs, 2:30pm-10:30pm **What You’ll Do*** Provide personal care and assistance with activities of daily living (ADLs), including bathing, dressing, grooming, oral hygiene, and toileting* Observe and respond to elders’ needs, promoting independence and decision-making* Maintain confidentiality and privacy of all elder care and documentation* Communicate effectively with coworkers and interdisciplinary teams to meet resident needs* Participate in Clinical Service Team (CST) meetings to discuss resident progress and needs* Document and report changes in health, behavior, eating habits, skin condition, and psychosocial status* Follow protocols for infection control, fire safety, disaster preparedness, and facility policies* Assist with safe transfers, positioning, and mobility support using appropriate equipment and techniques* Support restorative nursing and rehabilitation programs as outlined in care plans* Measure and document vital signs, weight, and height* Assist with meals, snacks, and fluids based on dietary plans and preferences* Maintain cleanliness and organization within the household, including resident rooms, shared spaces, and kitchen areas* Complete laundry tasks for residents including washing, drying, and folding* Follow safety protocols and report unsafe conditions or suspected abuse immediately* Participate in ongoing training and education to maintain compliance with state and federal regulations**What We’re Looking For*** Certified Nursing Assistant (CNA) listed on the Minnesota Registry* Strong communication skills with the ability to engage and support elders respectfully* Ability to work collaboratively within a team environment* Commitment to preventing abuse and neglect while supporting elder dignity* Ability to follow established care plans and safety procedures* **Preferred:** Experience in long-term care, assisted living, or skilled nursing settings* **Preferred:** Knowledge of electronic medical records (EMR)* **Preferred:** Experience in a household model of care or Green House home*\"At Episcopal Homes of Minnesota, we value a diverse, inclusive workforce and we provide equal employment opportunity for all applicants and employees.
