Job summary

Moms In Motion in Virginia Beach seeks a Service Facilitator to visit clients at home and guide families through Virginia’s Medicaid Waiver programs. You’ll coordinate plans of care and assessments, ensuring safe, healthy, independent living for those served.

Strong communication and basic tech skills are essential, and a valid driver’s license with a reliable vehicle is required. The role offers local travel and remote work from a laptop/tablet.

Qualifications

• Experience supporting individuals with disabilities or the elderly (minimum 2 years).
• No degree required; heart and experience prioritized.
• Strong communication skills and reliable transportation.
• Proficient with email, word processing, spreadsheets, and video calls.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Act as the go-to guide for families navigating waiver programs.
• Write care plans and assessments that actually make a difference.

What We’re Looking For

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.
