The Services Coordinator position actively participates in carrying out the mission of CCHC by providing direct support to tenants of a supportive housing apartment building serving homeless families. The Services Coordinator provides direct services as a member of a team and implements the program goals based on "Housing First" and "Harm Reduction" philosophies. This position will assist the Site Director with general provision of service coordination in the building including all aspects of tenant case management.
The Services Coordinator will work as a team member with CCHC staff to effectively meet the needs of the tenants. The Services Coordinator will assist the CCHC Site Director in the facilitation of services and to utilize the Harm Reduction/Housing First model. Services Coordinator must be able to work independently and effectively communicate with individuals and groups, advocate for tenants, complete progress notes and documentation as required, and maintain a good working relationship with colleagues and community agencies. This person must be able to accurately enter and interpret data required for reporting systems. This position requires flexibility in schedule. Degree in related field preferred with at least one year of experience working with homeless families. This position requires a valid driver's license and dependable transportation. This person must pass and maintain Minnesota Background Study clearance.
