Job summary

Aspire Living & Learning is seeking a Service Coordinator/QIDP to oversee active treatment services in ICF group homes for adults with developmental disabilities. You will lead development, coordination and monitoring of individualized program plans to promote independence and quality of life.

The ideal candidate has a bachelor’s degree in a human services field and at least 1 year of direct experience, with strong data-analysis and communication skills.

Qualifications

• Bachelor’s degree in human services related fields such as psychology, social work, special education, rehabilitation counseling, sociology, or a related discipline.
• At least 1-year direct experience working with individuals with intellectual or other developmental disabilities including experience supporting or implementing individualized programs.
• Ability to analyze data, identify trends, and make informed program recommendations.
• Demonstrated commitment to support individuals with dignity, respect and person-centered care with empathy, enthusiasm and passion.
• Excellent interpersonal, organizational and communication skills.
• Basic Computer skills, experience using Therap a plus!
• Must have satisfactory background checks
• Valid driver’s license and safe driving record
