Job summary

Rocky Mountain Human Services in Denver, CO is seeking a Service Coordinator to assist in planning, coordinating, and monitoring the Early Steps and Developmental Monitoring program services for children birth to five with developmental disabilities or delays.

The role schedules home visits, develops service plans, coordinates with providers, documents progress, and helps families access supports, advocating when needed to obtain services from community partners.

Qualifications

  • Bachelor’s degree in Early Childhood Education/related field or a minimum of five years related experience.
  • Ability to read, write and speak English fluently.
  • Excellent verbal and written communication skills.

Responsibilities

  • Schedules Service Plan meetings, documents progress toward established goals and assists in obtaining necessary services or supports.
  • Maintains appropriate documentation of plans and progress in developing necessary services or supports.
  • Ensures child specific information is documented and reported timely and accurately, and documents are uploaded and stored in the Rocky Mountain Human Services data system.
  • Helps RMHS implement agency goals and objectives according to the organization’s mission and values.
  • Develops and maintains productive relationships with families, service agencies, and others involved with the person.
  • Performs all job duties to established department standards related to quantity of work while ensuring high quality services and supports are provided to children and families.

Position Purpose

The Service Coordinator (SC) assists in planning, coordination and monitoring of the Denver Early Steps and Developmental Monitoring program services provided to children birth to five years old with developmental disabilities or delays and their families. SC schedule home visits, create service plans, find providers, document progress toward goals and assist the family in obtaining necessary services and supports. This position coordinates supports for families utilizing a variety of community resources and programs. They also assist the family through advocacy or mediation in getting what they need from various community providers (i.e., Denver Public Schools).

Essential Duties
  • Schedules Service Plan meetings, documents progress toward established goals and assists in obtaining necessary services or supports.
  • Maintains appropriate documentation of plans and progress in developing necessary services or supports.
  • Ensures child specific information is documented and reported timely and accurately, and documents are uploaded and stored in the Rocky Mountain Human Services data system (s).
  • Helps Rocky Mountain Human Services implement agency goals and objectives according to the organizations mission and values.
  • Develops and maintains productive and supportive relationships with the families, service agencies, and others involved with the person, as appropriate
  • Maintains the record as required by the Department of Human Services regulations and all funding agencies.
  • Participates in meetings and does any necessary follow-up to department agency assignments.
  • Completes paperwork to ensure continued eligibility for Target Case Management (TCM) services as applicable.
  • Performs all job duties to established department standards related to quantity of work while ensuring high quality services and supports are provided to children and families.
  • Provides intervention activities and administers developmental screenings for children birth to 5 in home or other environments.
  • Coordinates developmental screening opportunities for families within Denver City and County with several community partners
  • Preforms other administrative duties as assigned by leadership
  • Provides additional administrative support to other RMHS programs as needed

Minimum Qualifications
  • Bachelor’s degree in Early Childhood Education/related field or a minimum of five years related experience
  • Ability to read, write and speak English fluently

Preferred Qualifications
  • Bachelor’s degree in Early Childhood Education/related field
  • Knowledge of resources, policies, and history of EI systems at local, state, and national levels
  • Experience working with children ages zero to three
  • Two years case management experience working with people with developmental disabilities and their families
  • Ability to read, write and speak Spanish fluently
Driving Requirements

Employees who may drive RMHS or personal vehicles for business purposes. Category B employees may transport other employees but will not transport RMHS clients.

  • Valid driver’s license
  • Proof of motor vehicle insurance
  • Personal vehicle in good operating condition for use during work, including transporting individuals
  • No major violations in the past three years.
  • No more than two moving violations in the past three years
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
  • Drivers must upload proof of ongoing auto insurance every 6 months into Paycom/Relias.
