Job summary

Idaho Department of Health & Welfare’s Infant Toddler Program in Lewiston seeks a Service Coordinator to deliver case management for families with children birth to three. You will coordinate intake, IFSP development, and follow‑ups in diverse settings, including homes, daycares, and community contexts.

DEDICATED TO STRENGTHENING THE HEALTH, SAFETY, AND INDEPENDENCE OF IDAHOANS

The Infant Toddler Program is seeking applicants for a Service Coordinator to join the Division of Early Learning and Development; Infant and Toddler Program located at our Lewiston office. This is a position that provides case management to families of children birth to three in need of early intervention services. The individual who accepts this position will need to be able to work both autonomously and as part of a team. They must display reliability, excellent communication skills, attention to detail, and an exceptional ability to successfully coordinate and complete multiple tasks. Punctuality and dependability are key for this position, as well as the ability to work in the office and out in the field. Interactions with families will take place in a variety of natural environments, including family homes, daycares, and out in the community as well as through a virtual platform.

  • Work with interdisciplinary team to ensure referred children are evaluated for eligibility
  • Write Individual Family Service Plans (IFSPs) for eligible children
  • Conduct reviews of IFSPs at least every 6 months
  • Connect families to community resources and support
  • Complete data entry as required
  • Participate in required program trainings
  • Support families with transition out of the Infant Toddler Program, including to local school district programs
  • Collaborate with team members on shared cases

You must possess all the minimum qualifications below to pass the exam for this position.   • Good knowledge of human services principles and practices including behavioral management methods, social and family systems, cognitive behavioral education and case management methods. Typically gained by a certification as a psychosocial rehabilitation specialist in accordance with USPRA requirements OR bachelor's degree in social work, psychology, counseling, behavioral science, special education, psychosocial rehabilitation, healthcare, criminal justice or related field from a nationally accredited university or college OR 2-4 upper division courses as stated above AND 2-4 years’ work experience OR experience that represents the knowledge level above.
  • Computer literacy for data entry, electronic mail, composing reports and letters, and storing and retrieving documents.
  • A valid driver's license. Must be at least 21 years of age and meet eligibility requirements to drive a state vehicle.

If relocating to Idaho, the Department of Health & Welfare does not reimburse the cost of relocating and the department does not sponsor anyone with a work visa.
