Job summary

Idaho Department of Health and Welfare is seeking a PSR Service Coordinator for the Caldwell office to provide case management to families with children birth to three needing early intervention services.

The role requires autonomous and team-based work, strong communication, reliability and the ability to work in homes, daycares, communities and virtually. Training and collaboration with an interdisciplinary team are essential.

Qualifications

• Good knowledge of human services principles and practices including behavioral management methods, social and family systems, cognitive behavioral education and case management methods.
• Computer literacy for data entry, electronic mail, composing reports and letters, and storing and retrieving documents.

Responsibilities

• Provide case management to families of children birth to three in need of early intervention services.
• Coordinate and complete multiple tasks with reliability and punctuality.
• Interact with families in homes, daycares, community settings and via virtual platforms.
• Write Individual Family Service Plans (IFSPs) for eligible children.
• Collaborate with interdisciplinary team to ensure referred children are evaluated for eligibility.
• Conduct intake meetings with families and initial assessment.

The Infant Toddler Program is seeking applicants for a PSRService Coordinator to join the Division of Early Learning and Development; Infant and Toddler Program located at our Caldwell office. This is a position that provides case management to families of children birth to three in need of early intervention services. The individual who accepts this position will need to be able to work both autonomously and as part of a team. They must display reliability, excellent communication skills, attention to detail, and an exceptional ability to successfully coordinate and complete multiple tasks. Punctuality and dependability are key for this position, as well as the ability to work in the office and out in the field. Interactions with families will take place in a variety of natural environments, including family homes, daycares, and out in the community as well as through a virtual platform.

• Good knowledge of human services principles and practices including behavioral management methods, social and family systems, cognitive behavioral education and case management methods. Typically gained by a certification as a psychosocial rehabilitation specialist in accordance with USPRA requirements OR bachelor's degree in social work, psychology, counseling, behavioral science, special education, psychosocial rehabilitation, healthcare, criminal justice or related field from a nationally accredited university or college OR 2-4 upper division courses as stated above AND 2-4 years’ work experience OR experience that represents the knowledge level above.
• Computer literacy for data entry, electronic mail, composing reports and letters, and storing and retrieving documents.
