Job summary

Sheppard Pratt in Germantown, MD is seeking staff for a Crisis Residential Program. This direct care role involves assessing client needs, developing service plans, connecting clients to community resources, and advocating for clients to stabilize mental health crises.

The position offers a flat pay rate of $20.34/hr, opportunities for tuition reimbursement, comprehensive benefits, generous paid-time-off and flexible scheduling, with day/evening/weekend/holiday shifts as needed.

Qualifications

  • Combination of education and experience as listed.
  • Must be willing to obtain additional certifications and trainings.
  • Driver’s license with 3-points or less and access to insured vehicle.
  • Availability to work days, nights, weekends, and holidays as needed.

Responsibilities

  • Assess and monitor client needs and develop service plans.
  • Connect clients to community services and resources and advocate as needed.
  • Assist with enrollment/orientation and coordinate medical appointments.
  • Develop and coordinate transitions and program discharge.
  • Maintain accurate and complete documentation.

Education

High school diploma or GED + 2 years related experience
Bachelor's degree + 1 year related experience

Job description

Shift: | Job Location Type: In Person

Crisis Residential Program services provide short-term intensive, round-the-clock, personalized support to help stabilize someone having a mental health crisis so they can avoid hospitalization. Our teams collaborate with providers who are already involved in an individual’s treatment and works to link clients to other services offered by Sheppard Pratt or other community providers.

What to expect.

This is a direct care opportunity to support not just the mental health of your clients, but also their overall independence and well-being. You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.

Additional responsibilities will include:
  • Assisting clients through program enrollment and orientation.
  • Coordinating client medical appointments, transportation, housing, money management, etc.
  • Developing and assisting with client medication management plan.
  • Obtaining and maintaining entitlements for clients.
  • Maintaining accurate and complete documentation.
  • Developing and coordinating transitions plans and program discharge

What we need from you.
  • One of the following combinations of education and experience:
